School Nurse Resources

Welcome to the Clear Creek School Nurse Resource Page.  Here you will find specific forms that are used in the health offices at each school.  These will include health care plans for;  Allergies, Asthma, Diabetes and Seizures.  If your child will need access to any medications, including those that are self-carry and over-the-counter, there must be a physician order.

Allergy/Anaphylaxis Health Care Plan:
 

The link below contains the required form for any student who has a known or potential allergy that may require the administration of an antihistamine and/or Epinephrine.  It may also include an inhaler for a student with underlying asthma. 
 

As a reminder, all medications must be in the original container, labeled with the child's name and dosage of the medication.  Please also check medication expiration dates to assure they are not expired.

Asthma Health Care Plan:
 

The link below contains required form for any student with asthma and the need for an inhaler.  If the student is going to self carry an inhaler, towards the bottom of the page, the physician must check the box that self carry is permitted.  The student will also need to meet with the school nurse to confirm knowledge of the medication and correct administration technique.
 

As a reminder, all medications must be in the original container, labeled with the child's name and dosage of the medication.  Please also check medication expiration dates to assure they are not expired.

Diabetes Information:
 

If your child has been diagnosed with Diabetes we will require Diabetes Health Care Provider Orders/DMMP before student attends school. We will also set up a time to meet with your family to do the required intake form and get all procedures in place for your student.  
 

Attached please find the Colorado Standards of Care for Diabetes Management in a School Setting & Licensed Child Care Facility. We will follow your physician's orders and these standards of care.

Seizure Health Care Plan:
 

The link below contains the preferred form for a student with a seizure disorder who requires daily medications to be administered at school and/or emergency medications available in the event that a seizure occurs during school hours.

As a reminder, all medications must be in the original container, labeled with the child's name and dosage of the medication.  Please also check medication expiration dates to assure they are not expired.





 

Medication Authorization Form:
 

This is a general form for all prescription and over-the-counter medications that are not included in an allergy or asthma state of Colorado care plan.  Some physicians prefer to complete this form in addition to health care plans.

Remember, if your child needs any medication(s) during the school day, including all over-the-counter medications, such as Tylenol, Benadryl, and Advil, this medication authorization form must be completed and signed by both a physician and the parent/guardian. In addition, the medication must be brought to school in the original container, properly labeled with the student’s name.  Plastic bags are not acceptable. Also, please check medication expiration dates to assure they are not expired.
